LHCR Earns a Perfect Survey from the State of Ohio!

We are pleased to announce that the staff of the Lutheran Home at Concord Reserve was recently awarded Perfect Survey results on both our assisted living and nursing home surveys for 2014. The Ohio Department of Health conducts these surveys annually to evaluate many areas which include quality of care, nursing care, resident safety, and administration.

Our LHCR perfect survey results reflect the daily commitment of our staff and our medical director, Dr. James Wang, to continuously work to ensure our residents receive the quality care that they deserve.

Lutheran Home at Concord Reserve is an independent not-for-profit organization dedicated to providing excellence in health care, rehabilitation and senior living services, including Assisted Living. Concord Reserve is open to individuals of all faiths. We accept Medicare, Medicaid, Private Pay and several private insurances.
